Nomination Conditions
I
submit hereto the online
Nomination & Tournament Result Form which should be lodged if you wish to be
considered for selection in an Australian team to compete in the
2009 ITF World Seniors Teams Championships (age groups 35+ years to 55+years) or
the
2009 ITF World Super-Seniors Teams Championships (age groups 60+ to 80+ years
for men and 60+ to 75+ years for women).
In
2009 these Events and the World
Individual Championships will be held as follows:
Seniors Majorca,
Spain (Clay)
Teams April 27- May 02
Individuals May 03 -10
Super-Seniors Perth,
Australia (surface tbc)
Teams November 02
- 07
Individuals November 08 -
15

Selection Criteria has been reviewed and the updated Regulations are
available to be printed via the link at the top of this form or
click here.
Note that you only nominate for a Team in your Age Group as
determined by your age at the 31st December, 2009

The Player
Agreement and Release Form below must be carefully read and then I agree
typed below where required. Your nomination will NOT be accepted
without the completed Player Agreement and Release Form.

Your
nomination represents a commitment to compete. Please fully consider
all matters such as your physical fitness and health, family and business
responsibilities, leave from work, financial costs or any other personal
reason which might affect your availability at the time of the Teams
Competition. Any withdrawal after selection is a serious matter of concern
in the effect it can have on many other people.

If you intend
to compete in the ITF’s Seniors or Super-Seniors World Championships
immediately following the Teams Championships, you are obliged to meet your
full playing commitments in that event. Your return travel arrangements
must allow for this.

If you do not
have the current Australian clothing and Tracksuit please advise your
Secretary, Carl Anjou.

Late nominations will not be
considered under any circumstances after the closing date which is
12 midnight Wednesday December 10th 2008.

Player Agreement & Release
I wish to nominate for selection to represent Australia in the
ITF’s World Teams Championships as shown above.
I certify that:
1. I am eligible to represent Australia as I hold a current
valid passport and have not represented any other country during the 36
months which will precede the Cup for which I am nominating.
2. I am/will be of the age required by 31st December of the year for which I am
nominating.
3. I am a financial member of Division (State) of
Tennis Seniors Australia selected above
4. I am registered with State/Territory Tennis Seniors Association
selected above
5. I have fully considered all matters such as my physical fitness and
health, family and business responsibilities, leave from work,
financial costs or
any other personal reason which might affect my availability at the time of the
Cup Competition.
If selected I undertake to:
1. Cooperate fully with my Team Captain and accept all the decisions he/she
makes;
2. Wear the team playing uniform in all matches and other appropriate clothing
as advised;
3. Understand official team clothing provided or additional purchases made by me
is for my personal use only;
4. Accept the travel insurance where provided by Tennis Seniors Australia;
5. If competing in the ITF’s Seniors or Super-Seniors World Individuals
Championships immediately following the Seniors or Super-Seniors World Teams
Championships, to remain on-site and ready to play from the start of the
Competition and thereafter until the scheduled date
for completion of the events in which I enter or until eliminated there-from.
In submitting a nomination form I agree, as a condition of
nomination, that for myself and my executors, administrators, heirs and
personal representatives, all claims of any kind, nature and description
are waived, including past, present or future claims and injuries
if any sustained in travelling to or from or participating in the
competition, against Tennis Seniors Australia, the ITF, host nations and
sponsors of the competition.
